Recently, the Health Insurance Review and Assessment Service (HIRA) in Korea announced the new criteria and guidelines for appropriate antibiotics prophylaxis in surgery. Transurethral resection of the prostate, minimal invasive surgery of prostate using potassium-titanyl-phosphate (KTP) or holmium laser, simple prostatectomy and radical prostatectomy were included to this antibiotics prophylaxis assessment of HIRA in urological surgery. However, this criteria and guidelines have some problems such as neglecting the possibility of urinary tract infection (UTI) for antibiotics use, disagreement with other well-known clinical guidelines used in antibiotics prophylaxis of urologic surgery, and ignoring the patient-related risk factors affecting host response to surgical infections and/or UTIs. In this review, we compared current HIRA guidelines with other clinical antibiotics prophylaxis guidelines of urology and also analyzed the
anticipated problems in urological perspectives.
